Financial Assistance is provided to solid organ transplant patients who reside in one of the Northeast Florida counties of Duval, Baker, St. Johns, Nassau, or Clay.  Patients may contact their Transplant Case Manager (or Social Worker) for an application or click here if the patient is an adult or click here if the patient is a minor.  Typically, acceptable requests include basic living expenses such as rent, mortgage, utilities, and medication.  Other expenses will be considered, please call for more information.

The Nielsen Organ Transplant Foundation works with the local organ procurement organization to maintain current information regarding organ donation.  Brochures, speakers and other resources are available upon request.

Mentor Program

Our Mentor Program provides support to patients and caregivers who are just beginning their transplant journey.  The mentors have completed a day-long training workshop and are at least one year post-transplant.  We will match mentors and mentees based on several factors including language preference, illness leading to transplant, socio-economic background and more.  Click here for a Mentor Program referral form.
